Add questions to the rater form

The Open Items page allows you to add questions to the rater form. You can add up to five questions for raters to respond to in a short-answer format. Comments they make will appear verbatim in the report. You can select any combination of your own questions and the standard ones provided.

You can add, remove, or modify comments up until the first rater has completed the assessment, after which time the comments are locked in to ensure consistency.

To add questions to the rater form

1. On the Open Items page, click Add Item beside the items you wish to add.

   Clicking an item adds it to the Selected Items list.

2. To add a custom item, type it in the text box, then click Add Custom Item.
3. When you are finished, click Next to proceed to the Review page.
